Home First is a Care Quality Commission (CQC) regulated service provided by Herefordshire Council. The service is for anyone over the age of 18 who is eligible for social care support and who would benefit from a period of reablement. Home First is available for people who may have experienced:  An admission to hospital  A crisis at home  A temporary illness or permanent disability  A change in circumstances relating to staff or staff carer Reablement is a short period of rehabilitation. A period of reablement can help a person’s recovery by supporting them to develop their existing skills or learn new skills that are necessary for daily living. The Reablement ethos is different to that of a traditional care approach. Reablement will support a person to: • Regain skills and confidence in completing everyday tasks and activities such as making meals, washing and dressing. • Support staff to gain new skills to help staff manage at home. • Help avoid unnecessary admission to hospital, or long-term residential care. • Help staff get back on staff feet after a stay in hospital. The tasks you may be involved in - • Physical tasks: e.g. washing and dressing, end of life personal care, helping to toilet etc. • Domestic tasks: e.g. cooking, purchasing basic provisions. • Medication support e.g. eye drops, cream application, application of compression hosiery, prompting or administering medication. • Welfare – just checking • Moving and handling – e.g. using correct equipment to assist with all required transfers (i.e.: getting out of bed/ chair/shower).
